IV Drip Rate Calculation — Formulas & Methods

Accurate IV drip rate calculation is a fundamental nursing and clinical skill. Errors in IV flow rates can result in under-dosing, over-dosing, fluid overload, or electrolyte imbalance. This calculator supports all three common calculation scenarios in clinical practice.

Drops Per Minute (Gravity Sets)

For gravity-fed IV sets, the nurse must manually count drops and adjust the roller clamp. The formula is:

  • gtt/min = (Volume mL × Drop Factor) ÷ Time in minutes
  • Example: 500 mL over 4 hours with a 20 gtt/mL set → (500 × 20) ÷ 240 = 41.7 → 42 gtt/min

mL Per Hour (Infusion Pumps)

Modern infusion pumps are programmed in mL/hr. Drop factor is irrelevant for pumps. The formula is:

  • mL/hr = Volume mL ÷ Time in hours
  • Example: 1000 mL over 8 hours → 1000 ÷ 8 = 125 mL/hr

Infusion Time

If you know the rate and volume, infusion time is calculated as:

  • Time (hr) = Volume mL ÷ Rate mL/hr

Standard Drop Factors

  • 10 gtt/mL — Macrodrip, blood transfusions, large volumes
  • 15 gtt/mL — Macrodrip, standard IV fluids (some manufacturers)
  • 20 gtt/mL — Macrodrip, most common in general use
  • 60 gtt/mL — Microdrip, paediatric use, slow infusions, precise rates
